Smart Fields (Intake Forms & Charting Templates)
Automatically carry matching answers from a client's intake form into their charting template, so information like allergies, medications, or diagnosis codes only needs to be entered once. Smart Fields also sync select fields, like name, date of birth, and diagnosis codes, directly into the Client Profile and downstream billing forms. Field overlap between an intake form and charting template determines which fields sync.

The problem it solves
Providers and clients often re-enter the same information, like allergies or medications, across an intake form, chart note, and billing form, creating extra work and room for inconsistency. Smart Fields solves this by automatically syncing matching fields across those surfaces from a single source of truth.

How it works
A provider builds an intake form and a charting template using the Form Builder's question banks; if the same recognized Smart Field question appears on both, the client's intake answer automatically populates that field in the charting note. Some Smart Fields also sync beyond forms entirely, populating directly into the Client Profile (such as Referring Provider or Date of Birth) or into billing documents (such as ICD-10 and CPT code fields feeding a Superbill or CMS 1500). Editing the label of a recognized Smart Field triggers a warning, since renaming it can break fields that rely on it auto-populating elsewhere. Fields without an exact question match instead rely on Copied Fields, which pull the most recent value entered anywhere that field appears, rather than matching one specific intake question.

Some Smart Fields, like Client Name or Date of Birth, are recommended to leave unedited, since renaming them can break the sync.
