Chart Note Pre-fill
Automatically pre-fill a new charting note with content from a client's most recent note using the same template, so providers can build on prior documentation instead of starting from scratch. Enabled per charting template, not per client. Edits to the current note never change what was recorded in previous notes.

The problem it solves
Re-typing unchanged information, like an ongoing treatment plan or standing diagnosis, into every new chart note wastes time and risks inconsistency between sessions. Pre-fill solves this by carrying forward the client's last note on the same template automatically, so providers only need to edit what's changed.
What's included
- Per-template Pre-fill toggle in the Form Builder
- Automatic population of a new note from the client's latest note using the same template
- Isolation between notes, so editing the current note doesn't alter previous ones
- Combined behavior with Copied Fields, pulling from the most relevant prior note when both are enabled
- Fallback behavior for clients with no prior note using the template
How it works
A provider or admin enables Pre-fill for a specific charting template from Forms > Form Builder > Charting Forms. Once enabled, any new note created from that template for a client automatically populates with the content from that client's most recent note using the same template. If the template also has Copied Fields, and the client doesn't yet have a note using this exact template, those Copied Fields instead pull from the latest note that shares them, regardless of template; if the client already has a note using this template, Copied Fields pull from that note instead. Edits made to the new note apply only to that note going forward.
Chart Note Pre-fill automatically populates a new charting note with the client's most recent note using the same template, so providers can review and build on prior documentation rather than re-entering it. It's built for providers doing recurring, longitudinal care where continuity between sessions matters, such as reviewing a treatment plan or tracking symptoms over time. Pre-fill is toggled on per charting template from the Form Builder, not per client, so every note using that template inherits the setting. Editing the current note only affects that note; previously saved notes remain unchanged.
