The problem it solves
Field labels and placeholder text can shape what content goes into a field and how it's formatted, but they can't handle behavior that spans the whole note, like consistent clinical tone, comparing a value against a prior note, or keeping certain fields, such as billing or consent, untouched by AI entirely. Custom Instructions and Field Exclusions solve this by giving organizations a dedicated, prompt-level control surface for exactly that kind of cross-field and administrative behavior.

How it works
An admin or form editor with the AI Scribe organization permission adds Custom Instructions at the organization, appointment type, or form level; each instruction should target a specific field by name and state what to do affirmatively. These instructions are injected into Scribe's prompt at runtime, with form-level instructions overriding appointment-type-level, which override organization-level, whenever they conflict. Separately, any field in the Form Builder can be marked "Exclude Field from Scribe," which removes it from the AI's context entirely rather than merely instructing the AI to leave it alone; excluded fields are visually marked in the generated note and their original values are preserved. Field labels and placeholder text remain the first tools to try for field-specific content and formatting; Custom Instructions are reserved for tone, cross-field logic, and rules that can't live in a label or placeholder.

Custom Instructions are plain-language rules set at the organization, appointment type, or form level, injected into Scribe's prompt at runtime to control tone, clinical voice, cross-field logic, and specialty-specific documentation requirements; form-level instructions take precedence over appointment type, which take precedence over org-level. Separately, any Form Builder field, except a few unsupported types like iframe, image, and document, can be excluded from Scribe entirely via a checkbox, hiding it from the AI rather than just instructing it to skip. Editing either setting requires the "Can autogenerate charting notes by AI Scribe" organization permission.
