The problem it solves

Manually calculating gestational age during a visit is repetitive and introduces room for error, especially across a busy caseload of pregnant clients. The Gestational Age Calculator solves this by automatically deriving and displaying the value directly in the chart note based on the client's due date.

What's included

  • Gestational Age field available under the Charting question bank category
  • Automatic calculation based on a client's expected due date
  • Drag-and-drop placement into any charting template
  • Display of the calculated value directly within the chart note

How it works

A provider opens the Form Builder, selects a charting template, and switches the question bank category to Charting to locate the Gestational Age field. Dragging it onto the template adds it alongside any other charting fields. Once a client's expected due date is on file, the field automatically calculates and displays the corresponding gestational age whenever the chart note is opened or completed, without requiring manual calculation.

The Gestational Age Calculator is a charting form field that automatically calculates and displays a client's gestational age based on their expected due date, removing the need for a provider to calculate it by hand during each visit. It's built for maternal care providers and others working with pregnant clients who need gestational age reflected accurately in their documentation. The field lives in the Charting category of the Form Builder's question bank and can be added to any charting template alongside other fields.
