The problem it solves

Clients often struggle to verbally describe exactly where they're experiencing pain or want to focus care, and free-text alone doesn't capture spatial location clearly. Anatomical Charting solves this by giving clients a visual, front-and-back body diagram to mark directly, paired with a written description for additional context.

How it works

A provider adds the Anatomical Charting field to a form from the Form Builder, dragging it into place alongside any other questions, and can edit the field's title or instructions to clients. Clients see a front and back view of a body diagram and mark the relevant area(s), then use the accompanying text box to describe the issue in detail. Once submitted, the completed diagram and description save directly to the client's profile. Providers can distribute the form via a form request, add it to an Intake Flow for automatic completion, or set it to recur at a regular interval to track changes over time.
