Family health history, such as a first-degree relative's diagnosis, is clinically relevant but easy to lose track of if it's only mentioned in a chart note rather than stored as structured data. Family History Details solves this by giving providers a dedicated, structured place to document and maintain this information over time.

How it works
A provider navigates to Client Profile > Actions > Family History and selects Add Family History Entry to document a first-degree relative's diagnosis. Multiple entries can be added over time as more family history becomes relevant. To modify an entry, a provider selects the three-dot menu next to it and chooses Edit Family History Entry or Remove Family History Entry; removal requires a confirmation before the entry is deleted.
Family History Details let providers record, edit, and remove a client's first-degree relatives' diagnoses directly from the Client Profile, supporting ONC certification requirements around family health history. It's built for practices that need a structured place to capture hereditary risk factors as part of a client's overall clinical picture. A client can have multiple family history entries added over time, each independently editable or removable. Entries are managed from the Client Profile's Actions tab and are not visible to clients themselves.
