Demographics Details: Race, Ethnicity, Sexual Orientation, Gender, and Pronouns
Document a client's race, ethnicity, sexual orientation, gender, and pronouns on their profile, supporting ONC certification requirements for demographic and genetic-predisposition reporting. A separate "Sex on File for Insurance" field stays distinct from these fields to satisfy payer requirements on CMS 1500 claims. Gender Identity and Pronouns can be collected directly from clients via Smart Fields; Race, Ethnicity, and Sexual Orientation are entered by staff on the profile.

The problem it solves
Traditional demographic fields often don't capture race, ethnicity, sexual orientation, or gender identity with the nuance ONC certification and genuine patient-centered care require, and conflating gender identity with the sex designation insurance payers require creates both a documentation gap and a compliance conflict. This feature solves that by giving providers dedicated, ONC-compliant fields for each demographic category, kept separate from the insurance-required sex field.
What's included
- Race, Ethnicity, and Sexual Orientation drop-down fields on the Client Profile
- Gender field with an "Other" option that reveals a free-text Gender Identity field
- Separate "Sex on File for Insurance" field for CMS 1500 claim compliance
- Pronouns field with standard options and support for custom pronouns
- Gender Identity and Pronouns available as Smart Fields for any intake or charting form, with automatic profile sync
How it works
A provider documents a client's race, ethnicity, sexual orientation, gender, and pronouns from Client Profile > Actions > Client Info > Personal Information, selecting from each field's drop-down; Race currently allows only one selection. Selecting "Other" for Gender reveals a free-text Gender Identity field. To collect Gender Identity or Pronouns directly from clients, a provider drags those Smart Fields from the Client Info question bank onto an intake or charting form; when completed, the client's profile updates automatically. A custom pronoun can be added by selecting "Other" from the Pronouns drop-down and creating it as a new option. None of these demographic fields are visible to clients themselves.
Demographics Details let providers document a client's race, ethnicity, sexual orientation, gender, and pronouns from the Client Profile's Personal Information section, supporting ONC certification requirements around demographic data and helping providers understand genetic predisposition for holistic reporting. Race, Ethnicity, and Sexual Orientation are each selected from a dedicated drop-down (Race currently supports only one selection). Gender and Pronouns work the same way, with Gender offering an "Other" option that reveals a free-text Gender Identity field, and Pronouns supporting a custom entry. A separate "Sex on File for Insurance" field stays distinct from these fields since payers require a traditional selection on CMS 1500 claims. Gender Identity and Pronouns are also available as Smart Fields, so they can be added to an intake or charting form and will sync automatically to the client's profile; clients do not see any of these fields themselves.
