Care Teams
Associate multiple providers with a single client so they can coordinate care together, each with their own chat thread, shared access to intake forms, documents, and charting notes, and the ability to be selected (or not) when the client books an appointment. A client's Primary Provider remains distinct from other Care Team members, and Support-role staff can be added to a Care Team if given the right permission. Admins control per-member notification and scheduling settings for Care Team participation.

The problem it solves
Clients working with more than one provider need every involved provider to see the same forms, documents, and notes, and to be reachable individually, but a single Primary Provider field can't represent that. Care Teams solves this by letting a client be associated with multiple providers at once, each with full access and their own communication channel with the client.
What's included
- Multiple Care Team member association per client, alongside a distinct Primary Provider
- Shared access to intake forms, documents, and charting notes across all Care Team members
- Separate, per-provider chat thread for the client with each Care Team member
- Client-facing provider selection at booking, or a "no preference" option across the whole team
- Support-role Care Team eligibility, gated by a specific member permission
How it works
A provider adds Care Team members either when creating a new client, via the "+ Care Team Members" option, or afterward from Client Profile > Actions > Personal Information. Once added, that Care Team member has the same access as the Primary Provider to the client's intake forms, documents, and charting notes; if the client's organization uses a single shared intake flow, the client won't be prompted to redo it when a second provider is added. The client sees each Care Team member as a separate chat option, with a new thread created per provider, and any journal entries or metrics the client posts appear on every associated provider's home feed. When booking, the client can pick a specific Care Team member or select no preference to see combined availability across the team. Admins control whether a given team member's Care Team participation includes chat auto-creation, client scheduling ability (which requires "Should appear as a provider"), and activity notifications, all configurable per member.
Care Teams let providers add multiple team members to a single client's account, so more than one provider can coordinate on that client's care instead of working in isolation. It's built for practices where a client is seen by more than one provider, such as a therapist and a psychiatrist, or a dietitian and a health coach, and needs everyone involved to have access to the same information. Every associated provider gets full access to the client's intake forms, shared documents, and charting notes, and the client gets a separate chat thread with each Care Team member; the client can also choose which specific provider to book an appointment with, or select no preference to see all Care Team availability. Support-role staff can be added to a Care Team too, if they've been granted the "Can be added as a care team member" permission, and each Care Team member's notification and scheduling settings can be configured independently.
