The problem it solves
Providers need a way to communicate a treatment plan that a client can actually reference between sessions, since raw session or chart notes are written for other providers, not for patients, and aren't easy for a client to act on. Care Plans solves this by turning recommendations into a client-friendly, revisitable format that ties directly into goals, documents, and journaling settings the client already sees.

How it works
A provider opens a client's profile > Care Plans and selects either "New from Template" or "Create a Care Plan," then fills in a title, description, and any combination of recommendations, goals, documents, and journaling settings; recommendation fields can be pulled directly from a specific chart note or form response rather than retyped. Saving the plan as a Template makes it reusable, stored in a shared library and editable later from Settings > Care Plans > Templates, without those later edits affecting plans already active on a client. A Care Plan only becomes visible to the client once it's set to Active, which automatically deactivates whichever plan was previously active for that client; deactivating (rather than deleting) preserves the plan for later reactivation. Clients view their active Care Plan, tabbing through recommendations, goals, and documents, from the Care Plan section of their Client Portal or the Healthie mobile app, and get an email notification the first time a plan is shared with them.
