Scheduled Form Automations (Post-Booking, Pre-Appointment, Post-Appointment)

Automatically request form completion at three points tied to an appointment type: right after booking, a set time before the appointment, or a set time after it's marked as attended and occurred. Useful for waivers before a first visit, satisfaction surveys after a session, or any form tied to a specific appointment type rather than general onboarding. Configured per Appointment Type, separate from Intake Flows and Appointment Reminders.

The problem it solves

Certain forms only make sense in the context of a specific appointment, like a pre-visit waiver or post-visit survey, but manually remembering to request them for every booking doesn't scale. Scheduled Form Automations solves this by tying the form request directly to the appointment type's lifecycle, so it fires automatically at the right moment without staff intervention.

How it works

From an Appointment Type's Edit screen, a provider uses Schedule Content to attach a form to one or more of three triggers: After Booking, Before Appointment, or After Appointment, each with its own configurable time interval where applicable. The form itself isn't sent directly; the client receives an email prompting them to sign in and complete it. For the After Appointment trigger to fire, the client must first be marked as attended, and the appointment must then be marked Occurred; completing these steps in the reverse order will not trigger the request. Providers can view, edit, or remove any scheduled content from the appointment type's Actions menu.

Scheduled Form Automations let providers automatically request form completion at three points relative to an appointment: immediately after booking, a set interval before the appointment starts, or a set interval after the appointment is marked as occurred. It's built for forms tied to a specific appointment type rather than general onboarding, such as a consent form before an in-person visit or a satisfaction survey after a session. The After Appointment trigger requires both that the client be marked as attended and the appointment marked Occurred, in that order; reversing the order will not trigger the request. This automation is configured per Appointment Type and is separate from both Intake Flows and Appointment Reminders.
