Recurring Intake Forms
Automatically request the same form from a client or group on a recurring cadence, daily, weekly, monthly, quarterly, or annually, independent of any Program. Useful for periodic surveys, journal check-ins, or outcome tracking outside a structured program. Recurrence can be stopped or edited per client or per group at any time.

The problem it solves
Some forms, like a recurring symptom check-in or satisfaction survey, need to go out repeatedly over time, but manually re-requesting the same form on a schedule for every client or group doesn't scale. Recurring Intake Forms solves this by letting the request repeat automatically at a set cadence until it's stopped.
What's included
- Configurable recurrence frequency: daily, weekly, monthly, quarterly, or annual
- Support for targeting individual clients, entire groups, or both
- Selectable day-of-week for the recurring reminder
- Automatic date fallback when a monthly recurrence date doesn't exist in a given month
- Per-client or per-group controls to edit or stop recurrence, and to delete a not-yet-sent request
How it works
A provider sets up recurrence either from Forms > [form] > Actions > Request Form Completion, or from Clients > Groups > Actions > Request Form Completion, selecting the clients and/or groups and the desired frequency. Once active, the form is automatically requested again at each interval until the recurrence is stopped. To stop recurrence for a group, a provider uses Clients > Groups > Actions > Edit Recurring Forms; for an individual client, they navigate to the client's profile and use the status menu next to the recurring form, marked with a recurring-time icon, to edit or remove the recurrence. A future request that hasn't yet been sent can be deleted outright; once a request has gone out, only future recurrences can be stopped, since sent forms themselves cannot be deleted.
Recurring Intake Forms let providers request the same form from one or more clients or groups on a repeating schedule, without needing to build a Program to do so. It's built for use cases like a weekly check-in survey or a periodic outcomes assessment that isn't tied to a program's structure. Recurrence can be set at daily, weekly, monthly, quarterly, or annual frequency, and applied to individual clients, entire groups, or both at once; a specific day of the week can also be set for the reminder to send. If a monthly recurrence lands on a date that doesn't exist in a given month, the form sends on the last available day of that month instead.
