Journal Entries

Lets clients log food photos, water intake, activity, symptoms, bowel movements, and free-text notes directly from their Client Portal or mobile app, with each entry visible to their provider for comment and quick reactions. Providers control which entry types each client sees, on a global, group, or individual basis. Entries synced from a connected wearable are automatically tagged as device data.

The problem it solves

Providers need visibility into what a client is doing between sessions, eating habits, activity, symptoms, mood, without asking the client to remember and report it all verbally at their next appointment. Journal Entries solves this by giving clients an easy way to log this information as it happens, so providers can review and respond to it in real time.

How it works

A provider enables each entry type, Food, Water, Activity, Symptoms, Stool, and Notes, from Journal Settings at the global, group, or individual client level; every entry type is off by default except Food. Once enabled, a client sees the corresponding icon on their Client Portal dashboard or mobile app home screen, taps it, and fills in the relevant fields: a photo and hunger/mood ratings for Food, a quantity for Water, a type and intensity for Activity, one or more symptoms, a Bristol Stool Chart selection for Stool, or free text and an optional mood emoji for Notes. Wearable-synced entries (Apple Health, Google Fit, Fitbit) post automatically and are tagged to distinguish them from manual entries. Providers review entries from the Journal tab on their dashboard, filterable by entry type and client group, or from the Journal tab on a specific client's profile, and can leave a comment or a one-tap quick-reaction on any entry. Water and Stool logging are currently web-browser only; other entry types are available on both web and the mobile app.
