How it works
A file upload field is added to a form using the Form Builder. When a client completes that form, they're able to attach an image or document directly within it. The completed upload is saved alongside the rest of the client's form responses for the provider to review.
The File Upload field lets clients attach an image or document directly within a form, rather than submitting it through a separate channel. It's built for any provider using Healthie's Form Builder who needs to collect visual or document-based information as part of intake, charting, or program forms.
