A fixed default form that collects a client's insurance policy details and card photo during onboarding, pre-loading that data into the Client Profile and CMS 1500 claims. A client's "Use insurance" or "Don't use insurance" selection also automatically sets their Client Billing Type. Supports both primary and secondary policies, plus optional expiration notifications for the provider.

The problem it solves
Collecting insurance details manually, and re-entering them into claim forms, is slow and error-prone, and providers have no automatic way to know when a client's coverage sets their billing status or is about to expire. The Insurance Form solves this by capturing policy details once, syncing them into claims and the profile automatically, and tying the client's insurance choice directly to their billing type.

How it works
A provider adds the Insurance Form to an Intake Flow, or requests it ad hoc from a client's profile via Charting > Request Form Completion. A client completes the form by manually entering policy fields, or by photographing the front and back of their insurance card; manually entered fields pre-load into the Client Profile and any CMS 1500 form, while photographed cards are stored as images without populating structured fields. If the client selects "Don't use insurance," required insurance fields drop off the form, and their Client Billing Type is set to Self Pay; selecting "Use insurance" sets it to Insurance Pay. Providers can enable insurance expiration notifications from Settings > Personal > Notifications, which emails the assigned provider two weeks before a client's coverage expires.
