Behavioral Health Intake & Documentation
Gives Behavioral Health accounts access to Healthie's full specialty form library for intake and documentation, without building forms from scratch.
Built by Core Engineering
The problem it solves
Behavioral health practices need standardized intake and documentation forms — treatment plans, mental status exams, progress notes — but building industry-standard forms from scratch is time-consuming. Without a specialty-mapped forms library, practices have to source, format, and maintain these forms themselves.
What's included
- Access to Healthie's full library of 40+ Behavioral Health specialty forms
- Includes treatment plan, SOAP, and mental status evaluation charting templates
- Auto-scoring support for standardized assessments like PHQ-9 and GAD-7
- Forms appear automatically based on the account's specialty designation
- Additional forms available on request via Healthie support
How it works
Forms appear in the practice's Forms Library based on the specialty selected for the account, drawing from Healthie's Behavioral Health specialty form library — including forms like Behavioral Health Treatment Plan, SOAP Behavioral Health, Mental Status Evaluation, and auto-scoring PHQ-9/GAD-7 forms. Forms can be added to an intake flow for clients to complete electronically, or used by providers during charting.
Behavioral Health Intake & Documentation gives providers access to Healthie's full Behavioral Health specialty form library for intake and clinical documentation. It's built for behavioral health practices that need standardized, industry-recognized forms without building them from scratch. Forms become available based on the practice's specialty designation, and additional forms can be requested from Healthie support if something isn't already visible. This is a forms library, not a dedicated care pathway — there's no separate treatment protocol or workflow bundled with it.
