Substance use disorder programs need standardized intake and documentation forms, but building industry-standard assessment forms from scratch is time-consuming. Without a specialty-mapped forms library, practices have to source, format, and maintain these forms themselves.

How it works

Forms appear in the practice's Forms Library based on the specialty selected for the account, drawing from Healthie's broader Behavioral Health specialty form library. Forms can be added to an intake flow for clients to complete electronically, or used by providers during charting.

Substance Use Disorder Intake & Documentation gives providers access to a subset of Healthie's Behavioral Health specialty form library, mapped to Substance Use accounts. It's built for practices treating substance use disorder clients who need standardized intake and assessment forms without building them from scratch. Forms become available based on the practice's specialty designation, and additional forms can be requested from Healthie support if something isn't already visible. This is a forms library, not a dedicated SUD care pathway — there's no separate treatment protocol or workflow built specifically for substance use programs.
