Physical therapy practices need standardized intake forms and outcome measures — standardized tools that objectively track how a patient's condition affects daily function — but building industry-standard forms from scratch is time-consuming. Without a specialty-mapped forms library, practices have to source, format, and maintain these forms themselves.

How it works

Forms appear in the practice's Forms Library based on the specialty selected for the account, drawing from Healthie's Physical Therapy specialty form library, including standardized outcome measures. Charting also supports a body-map field that lets providers mark up an image of the human body to note areas of pain or focus. Forms can be added to an intake flow for clients to complete electronically, or used by providers during charting.

It's built for PT practices that need standardized, industry-recognized forms and outcome measures without building them from scratch. Forms become available based on the practice's specialty designation, and additional forms — including a provider's own existing forms — can be requested from Healthie support if something isn't already visible. This is a forms library, not a dedicated treatment-planning product — there's no separate care plan or protocol workflow bundled with it.
