Complete a Form or Intake Flow Outside Healthie (Website Embed & Sharing Link)
Generate an embed code or standalone sharing link for a single form or an entire Intake Flow, so prospective or existing clients can complete it directly from your website, email, or social media without logging into Healthie first. Completing the form automatically creates a new client profile for prospects, or attaches to an existing one by matching name and email. The problem it solves
Getting a prospective or existing client to complete a form usually requires them to already have, and log into, a Healthie account, which is friction a practice doesn't want on a public-facing intake or lead-capture form. This solves that by letting a form or Intake Flow be completed entirely outside of Healthie, automatically creating or matching the client record on submission.

How it works
A provider navigates to Forms, locates the form or Intake Flow, and selects Share & Embed to generate either an embed code (for adding to a website) or a standalone sharing link (for email, social media, or elsewhere). When someone completes the form, Healthie checks whether their name and email match an existing client; if not, a new client profile is created automatically, the provider receives a notification email, and the completed form is stored in that client's Charting notes. If the submitter matches an existing client, the completed form attaches to their existing profile instead. New clients created this way aren't assigned to a group, and don't receive a separate welcome email to activate a full account.

It's built for capturing prospective clients through a pre-screening questionnaire or survey, or letting existing clients complete an update request or feedback form without navigating into their portal. If a new person completes the form, Healthie automatically creates a client profile for them, and the completed form is stored in their Charting notes; an existing client is matched by name and email, and their submission attaches to their existing profile instead. Certain default forms, like Billing Info, Insurance Form, Photo ID, and Profile Picture, cannot be shared this way.
