Client Snapshot

Puts a one-sentence summary of who the client is at the top of their profile: age, gender, active diagnoses, allergies, current medications, and how long they have been a client. Providers get the clinical one-liner at a glance without scanning multiple sections of the chart.

Built by Core Engineering

The problem it solves

When a provider opens a client's profile, the basics they need to orient themselves are spread across separate sections for demographics, diagnoses, medications, and allergies. Assembling that picture takes several clicks on every visit, and in a handoff of care there is no single place that states plainly what is on record.

What's included

  • One-sentence summary combining age, gender, active diagnoses, allergies, active medications, client-since date, and last-seen date
  • Allergies and sensitivities always shown in full, including food intolerances, and never truncated
  • Explicit "No allergies on record" statement when none are documented, so absence is never ambiguous
  • Diagnoses and medications listed alphabetically, with the first three named and the remainder counted
  • Live updates when diagnoses, allergies, or medications change on the chart

How it works

Client Snapshot is enabled as a provider setting. Once active, it renders at the top of the client profile sidebar each time the profile loads. Age is calculated from date of birth, gender is read from the client's recorded gender identity, and diagnoses, allergies, and medications are pulled from the active entries on the chart, including both self-reported and e-prescribed medications. "Client since" reflects the date the chart was created and "last seen" reflects the most recent occurred appointment. Diagnoses, allergies, and medications are re-queried automatically when they change. Age, gender, and dates refresh on the next page load. The sentence follows clinical wording conventions, stating a client is "with" a condition rather than characterizing how they are managing it, and uses "on record" for all empty states. White-label organizations see their configured replacement term in place of "client."

Client Snapshot adds a single-sentence clinical summary to the top of the client profile sidebar, built directly from the fields already in the chart. It is designed for any provider who opens client profiles and wants immediate orientation before reading further. The summary draws from active diagnoses, active allergies and sensitivities, active medications, the chart creation date, and the most recent completed appointment. It updates automatically whenever diagnoses, allergies, or medications are added, edited, or removed.
